It is almost never a question to opt for the premium/severe service schedule. In this case, it is fancy oil change (oil, air, and cabin air filters and rotate tires, inspect all the other stuff) versus an actual service in which more fluids are changed (transmission & coolant) and belt as well.
The only item on that list I would question is the belt - let the mechanic use his or her judgement. If it's close to the end of its life, replace & be done, but if it's still plenty good, not cracked or glazed, etc, then don't worry about replacing it. Aside from that, changing those fluids at 30,000 is a MUST.
You'd probably pay about $100 for the standard service at one of our local shops (here in PA), or about $300 for the premium service. That's just a guesstimate on my part. Given the unique nature of the transmission (and I can't believe I'm actually recommending this), this type of service is probably best performed at the dealer (ugh, that was painful). Odds are, the dealership has their own service schedule they will try to sell you on - don't do it. Use the one in your owner's manual, it is there for a reason!
